I have had the tremendous privilege of serving the readers of the Journal of Gerontology: Medical Sciences as their Editor for the last 5 years. The Journal has been fortunate in receiving an extraordinarily strong set of articles from contributors. This has allowed the Journal to enhance its reputation as shown by the growth in its impact factor [Figure 1; see (1) for definitions]. Only the Neurobiology of Aging now has a better impact factor in the category of geriatrics and gerontology (Table 1). In addition, the Journal has the highest immediacy index.

Over the last 5 years, we have made a number of improvements to the readability of the Journal. This has included editorials that have concentrated on placing the original articles in the context of the developing field of geriatrics, as seen through the eyes of the Journal [e.g., (210)]. Occasionally we have tried to highlight the overall emerging areas in gerontology such as in the Top 10 Hot Topics in Geriatrics (11,12). This, for instance, allowed us to highlight the good, the bad, and the ugly of the rapidly emerging area of anti-aging medicine (1316). We introduced a series of review articles originally highlighting clinical topics, but, more recently, concentrating on some of the basic science foundations of geriatrics (1722). My own favorite has been the Future History series, where authors have skillfully blended the past with the future of geriatrics (23,24). We introduced target articles, accompanied by commentaries such as those on geriatrics as a chronic hospital specialty (25), to treat or not to treat hypertension in the old-old (2628), the role of insulin resistance in dementia (29), and the future of longevity (30). These articles certainly created a spirited response and dialogue (27,3137). Finally, the introduction of letters has allowed authors to defend their work (38,39).
From the 2001 list of citations (Table 2), Linda Fried and her colleagues' article giving an objective definition of frailty was the most highly cited article (40). This definition has now become the gold standard for research in the frailty arena. In 2004, Fried and colleagues updated the concepts of frailty, disability, and comorbidity in the Journal (41). In 2001, 16 of the 46 highly cited articles involved nutrition, 6 addressed hormones and aging, 6 physical activity, and 5 centenarians/longevity.
In 2002, 32 articles were cited 10 or more times (Table 3), and the review article on andropause was the most cited article (17). Testosterone replacement therapy in older males has become a major controversial issue in geriatrics, as shown by the numerous other articles on testosterone among the most cited list, and the fact that the Journal continues to publish articles on the testosterone controversy in 2004 (4244). Three further articles on frailty made the highly cited list for 2002. Anti-aging also began to become an important topic in 2002 (1316) and continues this year as shown by the series of anti-aging articles published in the Journal of Gerontology: Biological Sciences (4562) and in a full issue of Clinics in Geriatrics (6374) in 2004.
Sarcopenia began to appear as a major issue in 2002, and the series reviewing sarcopenia that appeared in the Journal toward the end of 2003 has already started to be highly cited (7582). In 2004, an article in the Journal suggested an important role for the vitamin D receptor genotype in the pathogenesis of sarcopenia (83).
For 2003, Blazer's review on depression and Wittert and colleagues' article on testosterone replacement have already been very highly cited (Table 4) (84,85). In 2003, 32 articles were cited 5 or more times. An extraordinarily important article from this year is the one by Colcombe and colleagues (86) highlighting the relationship of aerobic fitness to brain tissue loss. The series of articles on mobility is already beginning to be highly cited (8794).
Among the 3 years of citations, a number of articles have highlighted the growing importance of cytokines in the production of sarcopenia, frailty, and death in older persons (95). The Journal continues to publish articles in this area (96102). An editorial has characterized this occurrence as the Cytokine-Related Aging Process (103).
Articles within 2004 that are starting to occasion interest are the three on delirium (104106) and the highlighting of the role of the insulin resistance syndrome in aging (18,107). My own favorite article for 2004 was by Tamura and colleagues (108), which utilized an entertainment robot for the care of older persons with cognitive dysfunction. This is in keeping with the earlier emphasis in the Journal on putting the "home" back in nursing homes (109) and utilizing pet therapy (110) and the Eden Alternative (111). An innovation in 2004 was to begin to publish the Joseph T. Freeman award lectures (112). Finally, I trust that our readers obtained pleasure from the Brief History of Geriatrics and the oral (written) histories associated with it (113131).
My 5 years of stewardship of the Journal has provided me with tremendous enjoyment. I trust the readers have enjoyed the changes as much as I have. To maintain the quality of the Journal and the rapid review time (17 days to first decision in the first 6 months of 2004) has meant that a dedicated cadre of gerontologists have served as reviewers and on the editorial board. To these unsung heroes of journal production, I would particularly like to say thank you. My four associate editors, David Thomas, Joseph Flaherty, Larry Johnson, and Pam Cacchione, have provided extraordinary advice and managed the Journal on my frequent trips out of town. They deserve a hearty round of applause both from me and our readers. The staff in the Society's office, Patty Walker and Jennifer Campi, have provided solid support. My thanks also go to the Publications Committee members who have provided encouragement and support during my experimentation with changes in the Journal. My greatest gratitude goes to my assistant editor, Valerie Rincker. It is Val who saw that the manuscripts flowed smoothly. She was the foundation on which the Journal's success was built. From the bottom of my heart, I thank you, Val.
Finally, I congratulate Luigi Ferrucci on being selected as the next editor of the Journal. I trust that he, too, will be blessed with continuing to receive the outstanding quality of articles submitted to the Journal. The Journal is now truly the premier clinical geriatric journal. I have no question that Dr. Ferrucci will take the Journal of Gerontology: Medical Sciences to even greater heights. As the poet said, "Grow old along with me, the best is yet to be."
